Job Description:
The LPN/LVN administers bedside nursing care to terminally ill
patients, following the care plan and state Nurse Practice Act.
Provides skilled direct care, evaluates patient/family needs, and
supports the interdisciplinary team in care planning. Performs
treatments and medication administration under RN supervision,
monitors patient reactions, and reports changes promptly. Maintains
a safe, clean environment and accurate documentation of patient
progress. Establishes therapeutic relationships, encourages family
participation, and educates on illness, care, and discharge
preparations. Participates in on-call rotations, flexible
scheduling, and ongoing professional development. Requires
graduation from approved nursing program, valid licensure, at least
one year of relevant experience, and reliable transportation.
