all persons.Job DescriptionPURPOSE OF POSITION:As a nursing
practitioner in the Hemodialysis unit, she/he is engaged in direct
patient care. She/he develops and implements individualized nursing
regiment for the renal patient based on data collection, and
established standards of care.Responsibilities
- Performs nursing care responsibilities as specified in the
policies, procedures and protocol for the Hemodialysis Unit.
Attends regularly scheduled interdisciplinary meetings of assigned
primary patients. Understands and maintains the established
standards procedures and policies of this Institution.
- Executes prescribed dialysis regimen for each individual
patient, as assigned. Performs therapeutic measures, as delegated
and prescribed by the physicians; has a thorough knowledge of
medications and treatments being administered.
- Maintains, prepares and sanitizes equipment necessary for the
administration of Hemodialysis.
- Accurately records observations, treatment, medication and
other pertinent clinical information. Evaluates the nursing care
provided for patients; reviews and revises patient care plans.
Documents long and short-term goals and discusses goals with the
patient in collaboration and other disciplines.
- Keep Charge Nurse and Head Nurse informed of pertinent
information continuously throughout tour of duty.
- Maintains a safe and therapeutic environment for patients. Has
an awareness of the safety hazards in the unit and take
responsibility for corrections, when possible, Reports and records
incidents and accidents.
- Makes assignments and assumes the responsibility fort the
performance of team members through guidance and supervision of
their activities.
- Cooperates and collaborates with other staff in the
Hemodialysis Unit to maintain.
- Participates in the Performance Improvement program.
- May assume Charge Nurse Responsibilities when necessary.
- May be assigned other duties by Charge Nurse, Head Nurse.
- Guides and direct License Practical Nurse, Technicians and
Nurse Aids in rendering patient care.
- Maintains infection control protocol according to infection
control guidelines for Hemodialysis Personnel.
- Maintains clinical competency by attending and participating in
continuing education programs.
- In the absence of the technician, ensures daily check of the
water treatment system herein includes the RO units, sand, carbon
tanks, water softener, water pressure and temperature. Performs and
interprets testing for chlorine, chloramines and water ph.
Recognizes and reports abnormal findings to Head Nurse of Charge
Nurse.
- Documents and reviews with the patient monthly laboratory work
and URR in collaboration with other disciplines. Provides
appropriate patient teaching.

Renal FailureRenal OsteodystrophyMinimum Qualifications
- A valid New York State license and current registration to
practice as a Registered Professional Nurse in New York State;
and
- Holds, or obtains through facility orientation, a valid and
current certification in Basic Life Support (BLS) through the
