Job Description /n Job Description /n Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease. Provides nutritional assessment, education and counseling to individuals and/or groups. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility. /n Full time schedule working Monday-Friday from 8am-5pm. /n Address: 3003 New Hyde Park Road, Suite 409 - Division of Endocrinology /n Looking for candidates with vas knowledge in diabetic supplies and use of diabetic pumps. /n Job Responsibility /n /n - Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease. /n - Assesses nutritional needs and considers patients' cultural, educational, and learning issues. /n - Provides individualized nutrition education to patients, their families, and/or significant others based on their needs assessment. /n - Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility. /n - Communicates patient information to referring providers and appropriate staff in a timely manner. /n - Documents nutrition information discussed, written materials provided, and evaluation of patient’s receptiveness and comprehension of the education provided. /n - Performs related duties as required. Duties not mentioned here, but considered related are not essential functions. /n /n Job Qualification /n /n - Bachelor’s Degree in Food, Nutrition or Dietetics, required. /n - Registered Dietitian, required. /n - Current Diabetes Educator Certification, required. /n - Knowledge of diabetic supplies and diabetic pumps, preferred. /n /n OR /n /n - Bachelor’s Degree in Nursing, required. /n - Current license to practice as a Registered Professional Nurse in New York State, required. /n - Current Diabetes Educator Certification, required. /n - Minimum of three (3) years progressively responsible, related experience to ensure familiarity with scope of program. /n - Knowledge of diabetic supplies and diabetic pumps, preferred. /n /n This position will also require to take after hour calls for the physicians after the office closed. /n *Additional Salary Detail The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
