Experience with OASIS documentation and conducting Start of Care
visits required! Case management full time RN opportunities also
available throughout Ocean County. We are looking for exceptional

nurse you will be prepared to succeed and to deliver exceptional
care. Learn and expand your clinical skills caring for clinically
complex level patients in the home Perform 1-3 Start of Care visits
per day in a small geographical territory, focusing on 1:1 care
with each patient Deliver 1:1 skilled nursing, intervention, and
supportive care in accordance with individual care plan, including
administration of medication, wound care, treatments, and comfort
care Perform client assessments, provide education to the patient
and/or their family, and monitor patients' condition Qualifications
for a Start of Care nurse: A current NJ nursing license in good
standing. At least one year of related experience, preferably in
Home Health. Comfortable utilizing an EMR; experience with HCHB a
plus! Preferably experience with OASIS and completing Start of Care
visits. Additional Information: Base Pay, depending upon
qualifications and subject to negotiation: $55.00 - $62.00 per
point $5,000 Sign on Bonus for full time RN positions. As an
