Job Description Job Description Psychiatric Nurse Practitioner
We are seeking a compassionate, skilled, and dedicated Psychiatric
Nurse Practitioner (PNP) to serve as a long-term shift provider
within DHS facilities across New York City. This role involves
providing comprehensive mental health assessments, psychiatric
evaluations, medication management, and crisis intervention
services to individuals experiencing homelessness, many of whom
present with chronic mental illness, substance use disorders, and
co-occurring conditions. The ideal candidate will have experience
working in community psychiatry, be comfortable with underserved
populations, and demonstrate the flexibility and professionalism
needed to operate in a fast-paced, evolving environment.
Responsibilities: · Conduct psychiatric evaluations and mental
health assessments for shelter residents. · Develop and implement
individualized treatment plans, including medication management. ·
Provide short-term and long-term psychiatric care, including crisis
stabilization. · Collaborate with interdisciplinary teams (case
managers, social workers, primary care providers, DHS staff). ·
Maintain accurate, timely, and HIPAA-compliant documentation in
electronic health records (EHR). · Participate in case conferences,
quality improvement activities, and clinical supervision. · Refer
patients to higher levels of care or community resources as needed.
· Provide psycho education to clients, families, and staff
regarding diagnoses, treatment plans, and medication adherence. ·
Remain on-call for shift-based clinical needs as required
(depending on assignment site) Qualifications: · Current NYS
license and registration as a Nurse Practitioner in Psychiatry
(PMHNP-BC required). · DEA license and NPI number in good standing.
· Minimum 1–2 years of psychiatric experience, preferably in
community mental health or shelter-based settings. · Experience
working with homeless, marginalized, or high-needs populations
strongly preferred. · Excellent clinical, communication, and
organizational skills. · Ability to work independently and manage
caseload effectively during assigned shifts. · BLS/CPR
certification. Preferred Traits: · Trauma-informed care approach ·
Motivational interviewing and harm-reduction skills · Familiarity
with DHS systems or New York City public health infrastructure ·
Flexibility to work at multiple DHS sites across the five
boroughs
