Job Description Job Description RiseWell Community Services is
seeking a Part Time Assistant Case Manager who will report to the
Site Supervisor within the Outreach Services Department at Pilgrim.
In this role, you will assist Case Managers by providing selected
services to ICM clients. Our Outreach programs provide an array of
services designed to engage individuals who are reluctant to seek
help or experience barriers to access. Services are brought to the
person where they are located, whether that be at home, the street,
a hospital, or another location. As a part of this team, you’ll
assist those we serve to have their basic needs met by providing
referrals, linkage, and hands-on assistance with obtaining food,
employment, housing, and financial assistance. HOURS: 19 hours per
week, Monday through Friday QUALIFICATIONS: High School Diploma or
equivalent. Individual must have personal experience with a mental
health condition. Peer Certification preferred. Clean, valid New
York State driver’s license. Fingerprinting, criminal record check
and all necessary clearances from the Justice Center. At RiseWell,
